Attention New and Returning Staff!
In order to promptly process your information, we need you to submit ALL forms with appropriate accompanying documentation no later than the April 14th deadline.
What forms do I need?
American Citizens are required to complete and return the following forms prior to employment:
- Signed Contract
- Signed Personnel Practices Code
- National Background Check (minimum seven year residential history required)
- Medical Examination Form including PPD Test and immunization history
- Signed W-4 Tax Withholding Form
- I-9 Form & Supporting documentation
Newly hired employees must also include:
- Three completed references to be written and returned by the reference person.
- State Central Register Clearance Form
Foreign Citizens are required to complete and return the following forms prior to employment:
- Signed Contract
- Signed Personnel Practices Code
- Medical Examination Form including PPD Test and immunization history
- Signed W-4 Tax Withholding Form
- A copy of valid passport
- Social Security Card (to be arranged through the VISA process)
If you are under 18 years of age by your first day of employment, you must also have the following documents:
- Working papers (available at your school guidance office)
- A copy of your Social Security Card and Passport (if passport is not available, birth certificate)
- Permission to Leave Campgrounds
- Authorization for Emergency Medical Treatment
Parent signature is required on the bottom of:
- Personnel Practices
- Employment Application
- Employee Contract
